The answer I was given is that a goalkeeper should be booked if he rolls up his sleeves, whereas outfield players can do the same, but they won't get booked.
Someone did send me an answer saying that only keepers can be booked for holding on to the ball for more than 6 seconds, which is true. However, outfield players cannot handle the ball at all, so therefore cannot be booked for holding the ball for more than 6 seconds (they get booked for handball instead), whereas keepers are booked. Make sense ?
