I rented a car on my last 2 trips. It's a good option IMO to drive around the island and I have many friends in Kamala, Patong, Chalong, Nai Harn, so it is easier to me. Never had problem but I drive carefully for many reasons : Thai car drivers sometimes drive too fast or too slowly, breaking evry 5 meters, you have motorbikes everywhere as it has been said and if Thai people use to drive in Phuket, it is quite different for many farangs, sometimes drunk.

If you plan to stay in Patong, better you forget it as it is really difficult to find a place to park. I staid in nai nai road last time, most of the time I took a motorbike taxi to go to Bangla (20 bath) so I didn't care to be drunk, and sometimes I paked the car up soi Sensabai (in front of Bangla) for 40 bath/day.
