TM7 is the entry card you have to fill in that they stamp in and out fo the country.. Stapled into your passport..
For cert of residence take a copy of your lease agreement / tenancy / rental docs to Phuket town immigration.. 200 baht IIRC.. Used to do it in Patong but have stopped..
If you pay the guy in Patong you would need him to check the bike and submit the forms.. Will still need all the same forms tho..
I do know someone who can fix bent books.. Generally smooth over problems etc.. Obviously that costs more depending on what kind of problems..
