The top-up card is a scratch off card with a number that you send back by dialing *120*[Card Number]#.
All the instructions are on the Top-up card in Thai, but it is pretty obvious what to do. The same goes for the first time activation of the SIM.
You will keep using the same SIM card until it expires (time or money).
