I booked my travel at a TAT when I was there, in bangkok:
- Minibus to Pattaya
- Flight direct Pattaya to Phuket with Bangkok Airways
- Flight from Phuket to Bangkok
Total Cost: 7,300 bht

Break it up how you want but I think the minibus was 800baht.
