I was in Bangkok last September, and had to leave town after two nights!
Could not sleep, no breathable air and the temperature at night was the worst for years, even by thai standards!
And I had air-con!
In December was much better, some wind cleared the air, and the air was breathable.
But still prefer Phuket!

It's terrible! But not enough of a reason not to go there

If you have some pre-existing medical condition (eg. asthma) talk to your doctor beforehand and obviously take the necessary precautions when you get there.

I found the late-afternoon the worst. Humidity rises, more cars on the road, and whatever piddly breeze there was earlier in the day drops right away.

Cabs are cheap, the BTS is great, everything is air-conditioned.

I never had a problem either and I also read that air pollution has dropped 50% over the past several years. Bangkok is, however, still one of the more polluted cities in the world.

I lived in the Los Angeles area for 10 years and never had any problems there either, but on bad days people with respiratory conditions could suffer greatly. Any person with a respiratory condition needs to consider the fact that Bangkok may not the a good choice.

Those "masks" are worthless. At a minimum, bring an N95 compliant mask. The will give 95% protection from airborne particulate, mists and solids. They give no protection for gases such as Carbon Monoxide.
Example: Amazon.com: 3M 8210 N95 Dust Mask, Particulate Respirators 20 per Box: Home Improvement
