After exiting through Customs, go up to Departures level (4th floor) and get a waiting taxi. The fare is around 300baht and that includes tolls.
As long as you already have a hotel booked, you pay no commission to the taxi driver and if he/she approaches the hotel for commission, it doesn't involve you so why worry about it ?
If on the other hand you have not booked a hotel, then I would advise that you do so in advance. Plenty of recommendations on here.

Definitely book hotel in advance. Only an idiot or well seasoned traveler to Thailand doesn’t do it. When you depart immigration you will see a lot of Thai faces who are after you to transport you to your hotel. This can be overbearing for first time visitor. This must be said. If you keep your wits about you go straight upstairs to departures and catch a departing taxi. However this could also be confusing to a first time visitor as it will feel like a strange place. So I suggest look for the nearest Tourism Authority of Thailand (TAT) counter. They speak English and they will direct you to the taxi stand. Pick up a free map while you are there. Otherwise just turn right at after exiting immigration which will lead you to the taxi stand. In Feb I paid 370bt including tolls and tip.
If you feel compelled to take a private limo. It should cost between 600 and 800bt including tolls.
Look Bangkok is not third world and not all are out to scam you. Most are trying to earn a living; I have taken the private limos and got a reasonable service and no incident. They and taxi drivers will ask you what hotel you are staying at rather than do you have a hotel booked. But if you have not booked and you ask them to take you to a hotel then yes they will more than likely take you to one that will pay them a commission. So pre book. It’s easy to do.

It's pretty difficult to get a meter taxi these days lads....even when you insist at the dedicated taxi desk they still say 400bht + tolls...ask inside the cab and if they are busy they won't be interested
same same return...hard to get a meter taxi to airport...alsmost always ask for 400Bht now

Does this mean that they have stopped the taxis picking up outside Departures ? I was there last october (2007) and there were 50/60 all lined up looking for a fare back in to town. I insisted on the meter when I got in and it read "202" when I got out at my hotel near the Suk. I gave the driver 100baht for the tolls (70baht IIRC) and told him to keep the change, then 250baht for the fare (202baht) plus tip. OK, I paid 350baht but it works out less than 300 before tips.
As long as they haven't shifted those taxis outside Departures, then that is the cheapest option, as those guys hate going back in to town empty.
A tip for getting taxis from the hotel in central BKK to the airport, don't ask Reception or the Concierge, talk to some of the staff (bell boy, barman etc). They always know someone has drives a taxi and you can get them to arrange for you to be taken to the airport. This always works for me and I usually give the staff member 50/60baht.

From Suv to Suk getting a metre taxi to and from has never been a problem for me anyway.

If 350B is an OK fare and includes tip margin over the meter fare, should I just offer the driver 350B flat including tolls etc?

We arrived BKK 23/3/08 so no more than 3 weeks ago.
Walked outside arrivals and the touts wanted 800 baht from airport to new petchburi road.
Walked over to the taxi stand to the right and told driver I wanted to go to Classic Place hotel on the meter.
All up for the 4 of us was 300 baht plus 2 tolls one of 40 baht and 1 of 30 baht so 370 baht all up. Gave him 400 baht and he was happy.
Good price I think as in Melbourne that trip would cost a least $40 or approx 1120 baht.
Regards

In my infrequent visits to LOS I have had fixed and metered taxis. But if I was quoted 400bt including tolls I would take it. It is a fair price and if you get delayed with traffic and he takes "short cut", it could cost marginally more on a metre. But I dont tip on fixed prices.

I can't see any reason why not. But make sure he understands that tolls are included. I feel it's safer to pay tolls as you go and then the meter fare plus tip at the end.

Hey Guys, thanks again for the great info. I'm flying in 10 days and just needed some overlooked details. So I guess I go to the Taxi stand right of immigration or head out to the departure area upstairs for a decent taxi.

im in and out of bkk all the time
the safest option is to go to the booth, now outside arrivals both bkk and dmk, if you want to save 50 baht go to level 4 departures bkk, make sure you confirm they will take meter before you leave in the taxi, make sure he switches it on, tell him tollway and have change, just drip feed him the toll fares as he gets to the toll booth
I still have no problems getting meter taxis going into or leaving bangkok, either from DMK or BKK

Mr Toom seems to be well recommended. Just put his name in the search function and there will be plenty of threads about BKK - Pattaya transport.
Here is one thread

Since the taxis aren't officially allowed to pick up from departures, and get moved on pretty quickly, any money is good money for them. I usually offer 300Bht and that gets accepted pretty quickly.

no you are right Diz....you can go up to departures and get a meterd taxi who will be happy to take you back into town on a meter....well except if he gets his windscreen crackede by the airport mafia in doing so....
arivals is where I was talking about...at the offficial desk...they are now asking 400bht + tolls there...no option of meter

If it is your first time through Swampy ( the airport) then your best bet is going to be to go the taxi meter stand out the front of arrivals and get a metered taxi from there. There is no haggling or bargaining, it is a meter job with tolls added for the expressway. You can probably save 50 or 100 baht by going upstairs and picking a cab up at departures but probably not the best move on your first time through.
When you have finished with customs and immigration you will head towards the exit doors. Just walk through and out into the madness beyond. IGNORE all the taxi and limo and hotel touts and head straight for the nearest big glass door exit to the pickup area area outside. You will be able to see it from where you are inside. Out there will be a line of passengers ( dont worry if it is long, it moves really quickly) waiting to move towards a desk set on the pavement. This is the metered taxi stand. tell the girls which hotel you want to go to. They will write it down on a piece of paper and then you will be allocated a taxi driver who will look at the paper and grab your bags and away you go. The meter will be turned on before you start to move and if it isnt just tell him you want to get out and it will be turned on very very quickly.
The hardest part of the whole process is that initial shock when you come out of the exit door and into the madness of the greeting area. Just sail through as if you look like you know what you are doing and the touts will leave you alone and within 5 minutes tops ( less if you go through at a quiet time) you will be in a nice airconditioned taxi speeding towards your hotel.
