I never filled out any forms myself, always handled by the dealer. When I sold my bike once the Thai guy brought papers he already filled out and I just signed his papers to transfer ownership to him.
But this kind of staff is handled by transportation office, they have all forms you need. The same place where you apply for driver license (not same building though) and pay your taxes. Its located on one of the roads towards Saphan hin, close to labour office where you renew work permit.

You need..
Passport copy
Visa page copy..
TM7 copy..
Proof of address from immigration (only given in Phuket town a couple of months ago)..
You need the same (minus proof of address) from the seller if farang, or Thai ID card if Thai. As ever with Thai documents each page has to be signed by the person whose docs it is a copy of.
Plus the bikes green book.
You also need to have a sales transfer form signed by the seller and if the seller doesnt come with you Phuket town dept of motor vehicles then you also need a power of attorney form signed to say it can be done without thier attendance. Filled in in full with thier and your details (need a Thai reader)..
You go and do most form filling, then have to take bike for inspection (only pencil rub chassis numbers) then deposit all and books back with previous desk..
Next day (or is it 3 days ??) come back to collect book in your name.
If any of the visa's are out of date.. Or the seller isnt there to give ID etc it can result in a no transfer situation and the bike in that case becomes half price.. I know loads of guys riding round on bikes that cant move names cos the farang has left and no recent valid visa can be shown.

TM7 is the entry card you have to fill in that they stamp in and out fo the country.. Stapled into your passport..
For cert of residence take a copy of your lease agreement / tenancy / rental docs to Phuket town immigration.. 200 baht IIRC.. Used to do it in Patong but have stopped..
If you pay the guy in Patong you would need him to check the bike and submit the forms.. Will still need all the same forms tho..
I do know someone who can fix bent books.. Generally smooth over problems etc.. Obviously that costs more depending on what kind of problems..

For "Notification of stay" you need tabian baan from your landlord for the hourse/apartment you rent from, then they issue a paper that you need when dealing with vehicles.
When I bought my first motorbike, immigration provided me (of unknown adress) for 500 baht, second and third time I needed one it had to be my real adress.

I have never used a Tabien Baan.. I just take my lease agreement and get a slip that is a confirmation of address.. I am not listed on anyones tabien baan.. not sure what good it would do..
Trying to remember if I needed the Thai persons tabien baan if they were the seller ??
