First, used the search function and couldnt find an answer!!

So the question is, when you rent a car in Phuket is it insured or is it a bend it and mend it scenario as with bikes??

Reccomendations for good hire companies also appeciated.

Didn't fully go through that link again, but re insurance. I believe that only cars rented out by the reputable companies ie Hertz, Avis, Budget would have kosher insurance.

I agree with this.
Keep in mind that if you rent from a US corporation you are protected under US law. So if you trust, say Avis, in the US you can trust them equally in Thailand ... so long as the transaction involves the US entity; this will include making the reservation on the corporate web site.

The companies listed by Rodger generally includes full comprehensive and collision insurance as they require that for rental. On a previous trip I checked on local rentals which were a lot cheaper, however, by the time I added insurance I was within 10-15% of the major agencies.
